Under supervision, provides support, information and referral services for individuals utilizing programs in the community. This position will provide one-on-one support as well as group work in the areas of mental health, older adult services and/or those accessing services to basic needs. This position will be responsible for utilizing community resources, building effective relationships in the community and navigating human service agencies in the designated area. <br/>ESSENTIAL DUTIES AND RESPONSIBILITIES:<br/>This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time.<br/>*Provides support to individuals, advocates and assesses problems and establishes a positive rehabilitative influence to individuals and, where applicable, families. <br/>*Establishes and maintains effective community relationships. <br/>*Establishes and maintains files and records and may be responsible for tracking and reporting on various statistics and demographics.<br/>*May be responsible for coordinating and delivering presentation to communities based on program needs.<br/>*May be responsible for intake duties, involving receiving and interviewing individuals for community coordination. <br/>*May provide transportation for individuals to appointments or group activities as needed.<br/>*Participates in a team, ensuring individuals continue to receive increasing quality services.<br/>*May facilitate groups in the areas of life skills, recovery practices, socialization, education, employment, etc.<br/>*May works closely with human service and employment agencies to provide a wide range of outreach for programming and individuals. <br/>*May recruit and work with volunteers and interns to coordinate services<br/>*Performs other duties as required.<br/>QUALIFICATIONS<br/>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. <br/>EDUCATION AND/OR EXPERIENCE: <br/>*Graduation from high school or GED equivalency is required. <br/>*Bachelor's degree in Human Services, Social Work or closely related field is preferred. <br/>*One year of applicable work experience or one year of accredited training.<br/>*Working knowledge of welfare, health, mental health, older adult resources and other community resources.<br/>*Working knowledge of de-escalation techniques, trauma and crisis intervention.<br/>*Working knowledge of the federal, state, and local rules and regulations governing the delivery of social services.<br/>*Ability to explain service programs to individuals, families and others.<br/>*Ability to organize and plan work effectively.<br/>*Ability to accurately gather and record detailed information and facts.<br/><br/>CERTIFICATES, LICENSES, REGISTRATIONS:<br/>Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.<br/>TRAVEL: Occasional travel to meetings or program business in the community is required.
High School Diploma/ GED or better in Other.
Bachelor Degree or better in Hum. Serv. or Related Field.
