Now Hiring: Join Central City Concern's Pharmacy team as a Clinical Pharmacist

Location: Central City Concern - Old Town Clinic & Blackburn Center (2 primary locations)

Schedule: Monday - Friday (8:00am - 6:00pm)

MINIMUM QUALIFICATIONS (REQUIRED)

  1. Required: Bachelor's Degree (BS) in Pharmacy from an accredited pharmacy school.
  2. Required: Must be eligible for licensure, as a Pharmacist, in the State of Oregon, and obtained, prior to employment start date.

PREFERRED QUALIFICATIONS

  1. Doctor of Pharmacy.
  2. Bi-lingual (Spanish-speaking).
  3. PGY-1 residency training (or equivalent training or experience).
  4. Contemporary ambulatory pharmacy medication management and retail pharmacy operation management skills.

ABOUT THE CLINICAL PHARMACIST POSITION

SUMMARY: This position provides the services of a pharmacist to the Old Town Clinic and the Old Town Recovery Center. Old Town Clinic (a designated Health Care for the Homeless program) provides primary care to a population of low income adults with a broad range of physical, behavioral, and social conditions. The pharmacist must have the ability to collaborate with other healthcare providers and other pharmacy staff to provide high quality, patient-centered care that promotes outcomes which improve quality of life. The pharmacist will serve as a link between CCC pharmacy and Old Town Clinic providers. The pharmacist must be able to work independently, set priorities, make critical decisions, and respond to client concerns. The pharmacist will serve the patient by preparing medications, giving pharmacological information to the health care team, and monitoring patient drug therapies. The pharmacist will also be involved in the education and training of pharmacy students. Bi-lingual (English/Spanish) language skills preferred.

ESSENTIAL DUTIES and RESPONSIBILITES


1. Participate in the development of collaborative care protocols for several disease states, including but not limited to hypertension, hyperlipidemia, asthma, and COPD.
2. Participate in the development and managing of a Medication Therapy Management program.
3. Participate in the development of a Pharmacy and Therapeutics Committee, and perform drug usage and evaluation reports as needed for this committee.
4. Ensure safe, appropriate, cost-effective medication therapies for patients according to the established laws, rules, policies, procedures, and protocols.
5. Review medication orders for appropriateness and accuracy
6. Prepare and dispense medication orders per prescriber requirements according to established policies, procedures, and protocols.
7. Supervise and train pharmacy interns and externs. May serve as a preceptor as outlined by the State Board of Pharmacy.
8. Provide in-service training to pharmacy and clinic staff as appropriate.
9. Provide drug information service to providers and other healthcare professionals when requested or appropriate (e.g. cost-effective drug alternatives)
10. Report adverse drug reactions, drug defects, and medication errors according to procedure.
11. Collaborate and coordinate with care teams on patient care related to care protocols and medication therapy management.
12. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
13. Attend all mandatory CCC trainings in a timely manner.
14. Perform other duties as assigned.
